Short answer: As of October 15, 2026, ClinicalTrials.gov lists 21 Eli Lilly studies of eloralintide (LY3841136): 11 phase 1, 5 phase 2 and 5 phase 3, covering about 9,243 participants actual and planned. Seven are completed, three are active but not recruiting and eleven are recruiting. The five phase 3 ENLIGHTEN studies test eloralintide in obesity without diabetes, obesity with type 2 diabetes, sleep apnea, knee osteoarthritis pain and obesity that persists on a weekly incretin drug. Only the first-in-human study, the phase 2 trial and the EloraTZP phase 2b trial have reported results.

Key takeaways

  • 21 registered studies, all sponsored by Eli Lilly: 11 phase 1, 5 phase 2, 5 phase 3.
  • Phase 3 is five studies with 5,615 planned participants. Estimated primary completion runs from January 2028 (ENLIGHTEN-2) to June 2028 (ENLIGHTEN-6).
  • Specialist studies are running in liver and kidney impairment, Japanese and Chinese participants and women, which will shape any label.
  • Two phase 2 studies pair eloralintide with a second Lilly drug, macupatide (LY3532226), and a third phase 2 study tests eloralintide with tirzepatide (EloraTZP, finished).
  • Phase 3 studies list 115 to 185 research sites each, and all enroll adults aged 18 and over.
  • Registry dates are estimates and change. This page is a snapshot from the ClinicalTrials.gov API on October 15, 2026.

The program in numbers

Phase 1 Phase 2 Phase 3 Total
Registered studies 11 5 5 21
Participants (actual plus planned) 917 2,711 5,615 9,243
Completed 5 2 0 7
Active, not recruiting 2 1 0 3
Recruiting 4 2 5 11

The phase 2 total includes a multi-drug master protocol (LY900038, 1,481 planned participants) in which eloralintide is one of several investigational appendices, so the eloralintide-specific number is smaller. The counts come from a search of the registry for the code LY3841136, which finds records filed under that name.

Phase 3: the ENLIGHTEN program

Eloralintide studies by phase: 11 phase 1, 5 phase 2 and 5 phase 3 studies, about 9,243 participants actual and planned
Study NCT number Population Participants Status Started Primary completion
ENLIGHTEN-2 NCT07282600 Obesity or overweight with type 2 diabetes 1,035 (est.) Recruiting Dec 2025 Jan 2028
ENLIGHTEN-1 NCT07321886 Obesity or overweight without diabetes 1,980 (est.) Recruiting Feb 2026 Mar 2028
ENLIGHTEN-4 NCT07353931 Knee osteoarthritis pain with obesity or overweight 900 (est.) Recruiting Feb 2026 Mar 2028
ENLIGHTEN-3 NCT07369011 Obstructive sleep apnea with obesity or overweight 800 (est.) Recruiting Feb 2026 Mar 2028
ENLIGHTEN-6 NCT07392190 Persistent obesity on a weekly incretin drug 900 (est.) Recruiting Feb 2026 Jun 2028

All five are randomized, double-blind and placebo-controlled, with once-weekly dosing, and their primary outcome is the percent change in body weight. ENLIGHTEN-1, -2 and -6 each have four eloralintide dose arms plus placebo, and ENLIGHTEN-3 and -4 are master protocols with two sub-studies each.

Study Who qualifies (headline criteria) Extra primary outcome Sites listed
ENLIGHTEN-1 Adults with BMI 30 or more, or 27 or more with high blood pressure, dyslipidemia, sleep apnea or heart disease; stable weight for 90 days; at least one unsuccessful diet attempt; no diabetes None 158
ENLIGHTEN-2 Adults with type 2 diabetes on stable treatment for 90 days; BMI 27 or more; stable weight None 143
ENLIGHTEN-3 Adults with moderate to severe obstructive sleep apnea (apnea-hypopnea index 15 or more); BMI 27 or more; one sub-study for people unable or unwilling to use airway pressure therapy and one for people who use it Apnea-hypopnea index 115
ENLIGHTEN-4 Adults with knee osteoarthritis and BMI 27 or more; stable weight; at least one unsuccessful diet attempt; no diabetes WOMAC pain score 153
ENLIGHTEN-6 Adults on a stable weekly incretin drug who still have BMI 30 or more, or 27 or more with a complication (including type 2 diabetes) None 185

Common exclusions in the records include previous or planned weight-loss surgery or an endoscopic or device-based obesity procedure, and type 1 diabetes. The full criteria are on each registry page and change over time.

What each study is built to answer

  • ENLIGHTEN-1 is the main obesity study without diabetes. It has the longest follow-up, with study completion estimated for July 2030.
  • ENLIGHTEN-2 is the first to read out and covers type 2 diabetes, the same population as the EloraTZP phase 2b trial.
  • ENLIGHTEN-3 and -4 test whether eloralintide improves sleep apnea severity and knee pain as well as weight. Retatrutide’s TRIUMPH-1 reported results in those same two conditions.
  • ENLIGHTEN-6 asks whether eloralintide helps people whose weight has stopped falling on a weekly incretin drug, which is the add-on question.

Phase 2: five studies

Study NCT number Population Participants Status Started Primary completion
Master protocol LY900038 NCT06143956 Multi-drug phase 2 platform (eloralintide is one appendix; size covers all drugs) 1,481 (est.) Recruiting Nov 2023 Sep 2027
Phase 2 (Lancet) NCT06230523 Obesity or overweight without diabetes, versus placebo 263 Completed Feb 2024 May 2025
EloraTZP phase 2b NCT06603571 Eloralintide and tirzepatide, alone or combined, with type 2 diabetes 367 Completed Sep 2024 Jul 2026
Macupatide + eloralintide NCT07215559 Alone or combined, with type 2 diabetes 200 (est.) Active, not recruiting Oct 2025 Mar 2027
Macupatide + eloralintide NCT07589608 Alone or combined, without type 2 diabetes 400 (est.) Recruiting May 2026 Sep 2027

The master protocol’s participant count covers all its investigational drugs, not eloralintide alone.

  • The 263-person trial is the Lancet study, which reported -9% to -20% weight change at 48 weeks.
  • The 367-person trial is the EloraTZP study, which reported -23.3% for the highest combination at 48 weeks.
  • The two macupatide trials are testing eloralintide alone, macupatide alone and the pair, in people with and without type 2 diabetes. Primary completion is estimated for March and September 2027.

Phase 1: eleven studies

Phase 1, women NCT07808060 Female participants with obesity or overweight 34 (est.) Recruiting Sep 2026 Jun 2027

Phase 1 studies establish safety, how the body handles the drug and how it behaves in specific groups. Several in this list matter for a future label: the liver and kidney studies show whether people with impaired organ function clear the drug differently, the Japanese and Chinese studies check whether results hold in other populations and the female-only study (started September 2026) looks at a group underrepresented in earlier phases.

How to read a registry entry

  • Estimated or actual. Dates and participant counts marked estimated can change. “Actual” means it has happened.
  • Primary completion versus study completion. Primary completion is when data for the main outcome are finished. Study completion is the end of all follow-up. A company can file after primary completion.
  • Recruiting statuses. “Recruiting” means enrolling. “Active, not recruiting” means enrollment has closed and the study continues. “Completed” means it ended.
  • Phases. Phase 1 studies safety and pharmacology. Phase 2 looks for a dose and a signal. Phase 3 confirms results in large groups and supports a filing.
  • Master protocols. Some entries cover several sub-studies or drugs under one record.

How people find eloralintide trials

The registry at ClinicalTrials.gov lists every study and its locations, and Lilly’s trial site lists its own studies. Each record has a contact for each site. Anyone considering a trial should read the full eligibility list, understand that a study may assign placebo, and talk to their own clinician first. This page is not a recruitment channel, and Peptryn cannot enroll anyone or advise on joining.

What is missing from the registry

  • No adolescent or pediatric study. The five phase 3 studies set a minimum age of 18, and I found no study in people under 18 (I did not check each phase 1 record).
  • No head-to-head trial. No registered study compares eloralintide with semaglutide, tirzepatide, cagrilintide or retatrutide as the active comparator.
  • No cardiovascular outcomes trial. None is registered for eloralintide that I could find.
  • No oral formulation. All the registered studies I read use once-weekly injections.
  • No expanded-access program. None turned up in my search.

What to expect next
